EZ Reimburse Mastercard

To apply for the EZ Reimburse Mastercard, you have to have enrolled in a medical savings account during this past spring's open enrollment. The application is online. Please fax the completed application form to Fringe Benefits Management Company (FBMC), FRA administrator, ATTN: Enrollment Processing, at (850) 514-5806. Only faxed applications can be accepted.
