Grant Funded Faculty Assignments
The Office of Sponsored Programs needs to know when faculty whose appointments are either
partially or entirely grant funded plan to leave the university.
Faculty should complete the Sponsored Program Exit Questions and fax
the form to OSP at 3-2296 or send it via intercampus mail to the appropriate Team Leader at MSN 4C6.
