Life Insurance
Group Term Life Insurance will run through the end of the month in which you leave. If
you wish to convert this policy to an individual plan, please call Minnesota Life at (800) 441-2258.
Optional Life Insurance also runs out at the end of the month in which you leave. The vendor
will notify you of this once the vendor fails to receive a payment via payroll deduction. If
you wish to continue coverage in the form of an individual policy, you should contact your vendor
