To find a provider, please enter an address or zip code in the "Find a Dentist" search field on their website. The Commonwealth's plan is the DeltaPremier program. You may also call member services at 1-888-335-8296.

How to find your Delta Dental member ID

Delta Dental uses a different set of ID numbers from your medical insurance.

• • COVA Care or COVA HDHP: take your Anthem member ID but subtract FVI (#######XU)
  • COVA HealthAware take your Aetna member ID, subtract the two zeroes at the front, and add XU at the end (#######XU)
