Reminder for Submitting Performance Documents

The Acknowledgement of Extraordinary Contribution, which documents and recognizes staff for the extraordinary contribution they have made in the performance of their job duties, should no longer be submitted to the Workforce Planning team. Instead, please submit these forms (along with classified staff probationary reviews) directly to the Performance Management team at As a reminder, any classified employee who receives an overall evaluation rating of “Exceptional” must have a signed acknowledgement from on file with HR & Payroll, which should be submitted during the evaluation year and in conjunction with their annual evaluation.
